A joint capsule that tightened up and quietly took your range with it. A shoulder blade that stopped moving with the arm, which leaves the cuff doing work it was never built to do alone. And the loads you repeat, overhead at work, a bag always on the same side, a press you have been grinding out with borrowed mechanics.
None of that shows up in a quick glance. It takes a real, cause-first evaluation to sort out which one is driving your pain right now. That’s why a cortisone shot that faded and a band exercise off a printout keep falling short: they answer the symptom, not the source. Usually not, and it’s rarely the first step. Scans can show cuff wear and tear in shoulders that lift fine and never hurt, so an image on its own doesn’t tell you what is producing your pain. In Hawaii you can start physical therapy without a referral, so we begin with a hands-on, cause-first evaluation: where in the arc it catches, what the shoulder blade is doing, and what your cuff can actually hold. If imaging turns out to be genuinely necessary, we help you get it and coordinate with your doctor.
That call belongs to you and a surgeon, and we are not here to talk you out of it or into it. What we can do is find out how much of your problem is stiffness, weakness and mechanics, because that part answers to rehab. And if surgery is the right call, you go into it already knowing your team and already having a plan for the other side. We treat before and after surgery, and we coordinate with your primary care doctor, your orthopedic doctor and your insurance company.
You get a plan with an endpoint, and you hear it at the end of the first visit. Your evaluation is what sets the number, because a cuff that got angry last month and a capsule that has been locking down for a year are not the same job.
