First, relief
Pain
Turn the volume down.
We find what is provoking it and change what can change today, plus hands-on work so muscles that have been guarding get permission to stop.
What We Treat
Sitting through a whole movie. Your waistband by dinner. Sex. The appointment where you did not bring it up because you were not sure what to call it. Pelvic pain has causes you can find, and finding yours starts with a cause-first evaluation, not a guess.

Why It Happens
Your pelvic floor is muscle, and muscle holds tension the same way a jaw or a shoulder does. Hold it long enough and it stops letting go on command, then it starts hurting on its own. The nerves running through that area get more sensitive the longer that goes on. Hips, low back and the abdominal wall also refer pain straight into the same neighborhood. That is one reason a scan can come back clear while you are still in pain every single day.
Clear results do not mean nothing is wrong. It usually means the answer sits somewhere imaging was never going to show. Sorting out which of those things is driving it for you is the entire job of a cause-first evaluation. Dr. Simone Iriarte, PT, DPT, is the clinic’s pelvic floor physical therapist. She has worked with pelvic floor dysfunction for several years and continues to take courses in it. Pelvic floor therapy is one of the clinic’s own services. Care is one-on-one with a licensed physical therapist, every visit, so you explain this once, to one person.
The Honest Truth
Nobody regrets starting too early.
How We Fix It
You have carried this quietly, and you have carried plenty else besides. Every plan has an endpoint, and here is what the four phases look like when the pain is pelvic.
First, relief
Pain
We find what is provoking it and change what can change today, plus hands-on work so muscles that have been guarding get permission to stop.
Then, motion
Prime
Breathing, positioning and mobility work so the floor, the hips and the ribs move together instead of holding a brace nobody asked for.
Then, capacity
Perform
Strength through the hips, the deep core and the floor, built up to what your day actually asks: the desk, the drive, the lifting, the workout you quit.
Last, staying power
Prevent
We test it in the situations that used to set it off, and you leave knowing exactly what to do if it starts creeping back. That is the endpoint, not just another appointment.
Amazing group of professionals! They obviously care about the people they help and clearly know their stuff.

Phase 4, Prevent: keep it from coming back
Questions, Answered
Only as much as you want to. “It hurts to sit” is enough to start with, and whatever words you already use for it are the right ones. Nobody here is going to flinch, because this is ordinary work for a pelvic floor physical therapist. Care is one-on-one with a licensed physical therapist, every visit, so it stays a conversation between you and one person. You decide what happens at every step, and you can say no to any part of it.
No. Hawaii allows direct access to physical therapy, so you can book an evaluation yourself without asking anyone’s permission first. If you already have a doctor involved, we coordinate with them and with your insurance company. If you do not, that is fine too. Nothing about getting started requires you to explain this to a third party first.
That depends on what the evaluation finds, so we will not put a number on it before we have looked. What you get instead is a plan with an endpoint: four phases, Pain, Prime, Perform, and Prevent, each with a job and a way to know you have earned the next one. You will always know which phase you are in and what is left, rather than an open-ended string of appointments.
