Why It Happens

Elbow, wrist and hand pain is a symptom. Something causes it.

Your arm doesn’t hurt for no reason. And it’s almost never one thing. It’s usually a mix of four: a tendon at the elbow or the wrist irritated where it anchors into bone. A nerve getting compressed somewhere along its run, at the wrist, at the elbow, or higher up in the neck, which is why numbness can start nowhere near the spot that hurts. Grip and forearm muscles that lost their endurance and now work at their limit all day. And the sheer count of repetitions your day asks for: keyboard, tools, steering wheel, phone.

None of that shows up in a quick glance. It takes a real, cause-first evaluation to sort out which one is driving your pain right now. That’s why a brace, a squeeze ball and two days off keep falling short: they answer the symptom, not the source. Usually not to get started. A test can confirm a nerve is unhappy, but it doesn’t tell you where along the arm it is being squeezed, or what your grip, your wrist and your shoulder blade are doing to keep squeezing it. In Hawaii you can start physical therapy without a referral, so we begin with a hands-on, cause-first evaluation that traces the symptom from your neck to your fingertips. If testing or imaging turns out to be genuinely necessary, we help you get it and coordinate with your doctor.

Almost never, and total rest is rarely the answer anyway. A tendon that is never loaded gets weaker, and weaker tendons flare at lower loads than they used to. The work is figuring out which parts of your day are feeding the problem and which parts your arm can handle right now, then changing the first list and building up the second. That is decided with you, one-on-one with a licensed physical therapist, every visit, around the job you actually have.

You get a plan with an endpoint, and you hear it at the end of the first visit. Your evaluation is what sets the number, because an elbow that got sore last week and a hand that has been going numb every night for a year are not the same job. From there you move through four phases, Pain, Prime, Perform and Prevent, and you always know which one you are in and what is left. No open tab of appointments, and no vague “come back next week.”
