You booked. Good. Now here is the paperwork side, in plain language, so nothing about that first appointment is a surprise.

New patient forms
In preparation for your first appointment, please fill out our online new patient intake form (opens in a new tab).
We suggest arriving 10 to 15 minutes before the start of your first scheduled appointment to complete the registration process.
Referral and prescription forms
You do not need a referral to be seen. Hawaii has unrestricted direct access to physical therapy, which means you can see a licensed physical therapist for evaluation and treatment without a physician’s referral. More on that in the FAQ.
Some plans and some physicians still want a written referral or prescription on file. If yours does, use our referral and prescription form (opens as a PDF in a new tab).
You can type directly onto the form, but you will still need to print it out and have your physician sign it. Bring it to your next appointment with them.
What to bring to your first visit
- Your completed new patient intake forms, if you filled them out at home
- Your insurance card and a photo ID
- Any referral or prescription your physician has written
- Paperwork related to your medical history, including x-rays, ultrasounds and doctor’s notes
- Comfortable, loose clothing you can move in, and any brace or special shoes you already use
You will be one-on-one with a licensed physical therapist for that visit, and for every visit after it.
Insurance and billing
Coverage depends on your plan. Most plans cover some or all of the cost of physical therapy, but you should check with your provider to confirm your coverage and any associated fees.
What you pay at each visit usually covers only a portion of your current balance. We expect the rest to be paid by your insurance carrier, and you are ultimately responsible for the balance if your insurance does not cover it. If your insurance ends up covering more than expected and you overpaid, we issue a reimbursement.

We also coordinate with your primary care doctor, your orthopedic doctor and your insurance company, so the paperwork is not one more thing on your list.
The clinic offers online bill pay and a patient portal. Call the office and we will send you the link for your account.
