Why It Happens

Surgery fixes the structure. It doesn’t hand back the strength.

An operation repairs or replaces a part. What it can’t do is undo everything that happened around that part. Usually four things are working against you: the months of limping and guarding before the date, which cost you motion and muscle. Swelling and protective patterns after it, which teach the joint to move as little as possible. A chain above and below the surgical site that has been covering for it and now has its own habits. And the life you are going back to, the stairs, the shift, the grandkids, which is waiting at full difficulty.

That’s the work: reclaim the motion, rebuild the strength in order, and retest it under real load. We follow whatever restrictions your surgeon has given you and build everything else around what your body is actually doing today. We also coordinate with your primary care doctor, your orthopedic doctor and your insurance company, so nobody is working from a different page than you are.

Every plan has an endpoint, and you work it one-on-one with a licensed physical therapist, every visit. Here’s what the four phases look like before and after surgery.

First, relief

Settle it down and get it moving.

Hands-on work and carefully dosed movement bring swelling and pain down so the joint stops bracing, all inside whatever restrictions you have been given.

Then, motion

Prime

Win the motion back early.

Range comes first, and it is worth the effort, because motion you leave on the table early is the motion you fight hardest for later.

Then, capacity

Perform

Make it strong enough to trust.

Progressive strengthening for the repaired side and everything above and below it, loaded on purpose, so you stop treating that leg or arm like a guest.

Last, staying power

Return better than before.

Stairs, distance, lifting and whatever your life actually asks get tested under real load before you’re cleared to go all-in: that’s the endpoint, not just another appointment.

What people ask before booking for surgery rehab.

Usually yes. You go into an operation with whatever strength and motion you have on that day, and you start your recovery from there. Prehab uses the waiting time to hold onto range, keep the surrounding muscles working and get you comfortable with the movements you’ll be asked to do afterward. It also means the first post-op weeks are not the first time you meet your therapist. If your surgeon has given you limits already, bring them and we work inside them.

A sheet tells you what to do. It can’t tell you whether you’re doing it well, when to add load, or when the ache you felt last night means back off versus keep going. Those are judgment calls, and they are the whole difference between a joint that keeps progressing and one that stalls at fine. Here you make those calls with someone watching, one-on-one with a licensed physical therapist, every visit.
