Usually not, and it’s rarely the first step. Scans can show disc bulges and wear and tear in backs that have never hurt a day, so an image on its own doesn’t tell you what is driving your pain today. In Hawaii you can start physical therapy without a referral, so we begin with a hands-on, cause-first evaluation of how your back and leg actually move. If imaging turns out to be genuinely necessary, we help you get it and coordinate with your doctor. You don’t have to wait on a scan to start feeling better.
For most backs, the right movement is part of the fix rather than the risk. Hurt doesn’t always mean harm, and long stretches of total rest tend to leave a back stiffer and weaker, which sets up the next flare-up. What matters is the dose: which movements, how much, and when to add load. That gets chosen and progressed one-on-one with a licensed physical therapist, every visit, so nothing moves faster than your back is ready for.
You get a plan with an endpoint, and you hear it at the end of the first visit. Your evaluation is what sets the number, because a back that flared last week and a leg that has been burning for two years are not the same job.
