A nerve root irritated where it exits, referring pain into a shoulder blade, an arm, a couple of fingers. Deep neck and shoulder-blade muscles that lost their endurance, so your head drifts forward and the small muscles hold it there all day. And the hours themselves, a monitor set too low, a phone read in your lap, a pillow that stopped working.

Usually not, and it’s rarely the first step. Imaging can show wear and tear in necks that have never hurt a day, so a picture on its own doesn’t tell you which structure is producing your symptoms. In Hawaii you can start physical therapy without a referral, so we begin with a hands-on, cause-first evaluation of how your neck moves and what reproduces the pain. If a scan turns out to be genuinely necessary, we help you get it and coordinate with your doctor. You don’t have to wait on imaging to start feeling better.

Nothing happens to your neck that you haven’t heard explained first. Your evaluation screens what is and isn’t appropriate for your neck, and the hands-on work is chosen to match it, one-on-one with a licensed physical therapist, every visit. You are told what is being done, why it is being done, and what it should feel like before it happens. If something doesn’t feel right, you say so and it stops.

You get a plan with an endpoint, and you hear it at the end of the first visit. Your evaluation is what sets the number, because a neck you slept wrong on and a nerve that has been buzzing into your hand for a year are not the same job. From there you move through four phases, Pain, Prime, Perform and Prevent, and you always know which one you are in and what is left. No open tab of appointments, and no vague “come back next week.”
