Why It Happens

Leaking is a symptom. Something causes it.

Your pelvic floor is muscle. Muscle can be weak. It can also be too tight to let go, or strong enough but late, closing a half second after the cough already happened. Pressure from above is part of it too: how you breathe, how you brace, and whether your hips and deep core take any of the load when you lift, laugh or run. Leaking is one of those things going wrong. It is not age, and it is not something you brought on yourself.

Which one it is changes the whole plan, and none of it is obvious from the outside. That is why more squeezing is not automatically the answer: a floor that is already gripping all day does not need to grip harder. So the evaluation comes first. Dr. Simone Iriarte, PT, DPT, is the clinic’s pelvic floor physical therapist. She has worked with pelvic floor dysfunction for several years and continues to take courses in it. Pelvic floor therapy is one of the clinic’s own services, not something bolted on.

What people ask before they book.

Only as much as you want to. “I leak when I cough” is enough to start with, and whatever words you already use for it are the right ones. Nobody here is going to react, because this is ordinary work for a pelvic floor physical therapist. Care is one-on-one with a licensed physical therapist, every visit, so it stays a conversation between you and one person. You decide what happens at every step, and you can say no to any part of it.

No. Hawaii allows direct access to physical therapy, so you can book an evaluation yourself without asking anyone’s permission first. If you already have a doctor involved, we coordinate with them and with your insurance company. If you do not, that is fine too. Nothing about getting started requires you to explain this to a third party first.

That depends on what the evaluation finds, so we will not put a number on it before we have looked. What you get instead is a plan with an endpoint: four phases, Pain, Prime, Perform, and Prevent, each with a job and a way to know you have earned the next one. You will always know which phase you are in and what is left, rather than an open-ended string of appointments.
