Why It Happens

It’s usually some mix of four culprits: an original driver that was never actually found, so it kept getting reloaded. A nervous system that stayed on alert long after the tissue settled, turning the volume up on ordinary movement. Strength and motion that quietly drained away during all the months you spent avoiding. And a daily pattern of guarding, bracing and holding still that keeps feeding the loop.
None of that shows up on a scan, which is exactly why so many people leave imaging with a normal result and no plan. It takes a real, cause-first evaluation to sort out which of those is driving your pain today, in what order to address them, and what your body will actually tolerate right now. That’s a fair question, and the honest answer is in how the hour is spent. A lot of people land here after somewhere handed them a printout and a heat pack. Here, you get a cause-first evaluation, an explanation of what we found in plain language, and your own progression, one-on-one with a licensed physical therapist, every visit. Not a room of people doing the same exercises. If a previous plan never named a cause, that is usually the reason it never ended.
No. Your pain is real, and a clean scan is not evidence against it. Imaging shows structure. It does not measure how a joint moves under load, how much a muscle can tolerate before it complains, or how sensitive an area has become after months of guarding. Those are the things we test by hand, and they are the things that change with treatment. Being told nothing is wrong when something clearly is has a way of making people stop asking. Keep asking.
How long you’ve had it changes the plan, not whether the plan can work. Tissue responds to load at any age, and a system that learned to guard can learn to stop. What a long history does mean is that we start smaller and build more deliberately, so progress sticks instead of setting off a flare. You’ll still get a plan with an endpoint and four phases, Pain, Prime, Perform and Prevent, so you always know where you are in it.
