We screen before we touch anything.
One quick, controlled input at one stiff segment. Screened for safety first, and always paired with the strength work that keeps the change.
What It Is
A segment of your spine can stop moving the way the ones above and below it do. You feel that as a stiff band, a catch when you turn your head, or a back that never quite settles no matter how you sit.
Spinal manipulation is a fast, small, controlled push applied to one of those segments, at the end of the motion it already has. It is over in about a second. You may hear a pop. That sound is gas moving inside the joint, and it is not the measure of whether anything worked.
Physical therapists in Hawaii perform spinal manipulation, and here it is screened before it is used: your history, your symptoms, and the tests that say whether this is the right input for you. If the screen says no, we do something else and tell you why. If it says yes, it is one input inside your plan, never the plan itself, and it is always paired with strengthening. A joint that moves better still needs something to hold it there.
Always optional, never sprung on you
What It Helps
It comes up most when one segment has stopped moving and everything around it is working overtime to cover for it.

Questions, Answered
Screening is the first step, not an afterthought. Your therapist takes your history, checks your symptoms, and runs the tests that say whether your spine is a candidate for this at all. If anything says no, we do not do it, and you hear why. When it is a yes, it is one quick input at one segment, over in about a second. Most people describe a brief pressure and then less of it. The pop you might hear is gas moving in the joint.
The hands-on technique overlaps, and chiropractors do good work with it. What differs here is what surrounds it. Physical therapists in Hawaii are trained and licensed to perform spinal manipulation, and we use it as one input inside a physical therapy plan: screened first, applied to the segment your evaluation identified, then followed by the strength and movement work that holds the change. If you are already seeing a chiropractor, tell us and we will factor that in.
This is not a standing appointment you keep forever. Your evaluation sets the length of the plan, and the plan has an endpoint. Manipulation is used on the visits where it is indicated, one-on-one with a licensed physical therapist, every visit, and the strengthening half is never optional, because the goal is a spine that keeps its own motion without the input.
