Why It Happens

The load is changing. The plan should change with it.

Your body is doing something big on a schedule. Weight moves forward. The ribs and the pelvis make room. Ligaments give a little more. And the muscles that used to manage all of that are working at a slightly different angle every few weeks. Pain in the back, the hips or the front of the pelvis is usually that load landing somewhere that has not been asked to take it before. It is not a sign that you are doing pregnancy wrong.

It is also worth sorting out while it is happening, not after. A cause-first evaluation finds where the load is actually going, and the plan moves as the pregnancy moves. Dr. Simone Iriarte, PT, DPT, is the clinic’s pelvic floor physical therapist. She has worked with pelvic floor dysfunction for several years and continues to take courses in it. The clinic runs pelvic floor therapy and women’s health as its own services. Care is one-on-one with a licensed physical therapist, every visit, so what you are doing this month gets adjusted for next month.

What people ask before they book.

No. Hawaii allows direct access to physical therapy, so you can book an evaluation yourself and tell your provider before or after, whichever you prefer. If you want us coordinating with them, we do that, along with your insurance company. If anything we find belongs on your doctor’s desk, we say so and we send it there.

There is no window you have to hit. The work simply changes depending on where you are, which is the whole reason we evaluate first instead of handing you a printout. Earlier on it tends to be about comfort and building something to carry the next stretch with. Later it shifts toward positions, breathing and what your body is going to be asked to do. And if you are reading this after the birth, that is not too late either.

That depends on what the evaluation finds, so we will not put a number on it before we have looked. What you get instead is a plan with an endpoint: four phases, Pain, Prime, Perform, and Prevent, each with a job and a way to know you have earned the next one. You will always know which phase you are in and what is left, rather than an open-ended string of appointments.
